Condition Care Management: Obesity

Magellan Condition Care Management: Obesity uses a unique four-tier program to provide a complete suite of tools for successful weight management matched to the degree of support needed by the individual.

Tier One: Web Entry and Resources

On MagellanHealth.com, animated coaches engage members and guide them though a host of weight management resources and screening tools aimed at assessing their needs and providing individualized and interactive nutrition and exercise programs.

Members who will benefit from the second and third tiers of intervention are identified through these animated coaches and screening tools. Magellan uses statistically based criteria and works with our customers to establish “thresholds for participation” that meet the financial, clinical and philosophical needs of the plan, allowing us to move only the most appropriate members from the Web to the more intensive components of the program.

Tier Two: Telephone Consultation

Members are assigned a dedicated coach who will work with the member, one-on-one, to provide education, tools and guidance to help members build the skills necessary to achieve their goals and make long-lasting behavior changes.

Our program offers a range of call models to maximize education, feedback, support and encouragement for the member. Magellan provides guidance on using the Web site more effectively, identifies community-based programs, and focuses on behavioral change techniques.

Tier Three: Network Support

Magellan has developed a network of thousands of psychiatrists, psychologists and social workers nationwide who have the requisite training or experience for inclusion in this program. These experts offer intensive behavior modification therapy and tailored exercise and nutrition counseling.

In consultation with independent industry experts, Magellan has structured a training program for this specialty network to assist providers in working with member self-assessments and clinician-directed tools that focus on medical issues, weight, diet, nutrition and eating behaviors. This tier also emphasizes the importance of understanding the risks, benefits and lifelong implications of bariatric surgery.

Tier Four: Bariatric Surgery Support

Designed for members who have met the criteria for and asked to have bariatric surgery, this intensive assessment and management program helps ensure that only those candidates who are medically cleared, fully informed and emotionally ready to undergo this life-changing procedure are recommended.

Magellan then supports these members through the surgical period and in the post- operative phase via telephonic coaching, network visits and group therapy. Our program spreads out these elements during post-op to support the members when they need it most.
